Our family forms

Two short forms help us build the pack around your family. Print, fill out, and hand either one to any pack leader at a meeting.

Required forms

Health & Medical Record

Scouting America requires every Scout and every participating adult to have a current Annual Health & Medical Record (Parts A & B) on file with the pack. It’s a parent-completed form — no doctor’s visit needed for typical Cub Scout activities — and it’s good for 12 months. We collect them each August.

  • Annual Health & Medical Record (Parts A & B) Download the current form, instructions, and FAQs directly from Scouting America: scouting.org/health-and-safety/ahmr
  • Turn it in Print, complete, and hand Parts A & B to your den leader or any pack leader at a meeting. Attach a copy of your insurance card (front and back).

Set up once, use all year

  • my.scouting.org — national registration Create your family’s Scouting America account and register your son: my.scouting.org
  • BeAScout — find and apply to Pack 226 Search for Pack 226 (Longhorn Council, Bedford, TX) and apply online: beascout.scouting.org
  • Scoutbook — track your son’s progress Free advancement tracking for every registered Scout. Log in with your my.scouting.org account: scouting.org/resources/scoutbook

The Cub Scout Six Essentials

Every Scout carries these in his backpack on outdoor adventures:

  • Refillable water bottle
  • First aid kit
  • Whistle
  • Flashlight (with spare batteries)
  • Hat & sunscreen
  • Trail snack
Campout packing list

What to bring on a pack campout

Scout items

  • Scouting uniform (Class A button-up)
  • Pack t-shirt (Class B)
  • Scout handbook
  • Pens, pencils & notepad
  • Bible
  • Six Essentials (see above)

Sleeping & clothing

  • Tent, sleeping bag & pad or cot
  • Pillow & dry bag
  • Layers for the weather + rain poncho
  • Extra socks & underwear (1.5 per day!)
  • Hiking boots or sturdy shoes
  • Laundry bag

Other

  • Toiletries & wash cloth
  • Insect repellent
  • Plate, cup, bowl & utensils (optional)
  • Medical form & any prescriptions (in original bottles)
  • Fishing gear (when the trip calls for it)

Please leave at home: pets, electronics, sheath knives, hatchets, fireworks, and anything else that would make the Cubmaster’s hair grayer than it already is.
